Why this is here
A25: “A correction is recorded ⇏ The corrected capacity is maintained.” This is the report it leaned on. Bears on CASE-138 and CLAIM-422.
Corrected 2026-10-22, from the reading. The report does not concern Challenger or the corrections made after it. It was prompted by the losses of the Mars Climate Orbiter and Mars Polar Lander in 1999, and it audits the Lessons Learned Information System, created in 1995; Challenger appears only as one column of a RAND table. What it shows is a written requirement to apply lessons, practice that did not follow, and an agency that measured searches of its database and not the application of lessons. That supports CLAIM-422’s separation of procedure from capacity, as a recorded lesson not applied, rather than A25’s reading of a correction that held and then decayed. In CASE-138 it is evidence of the same gap at the same agency before Columbia, not of the post-Challenger reforms eroding.
