Why this is here
The owner proposed escalating responses under the National Incident Management System, and a fireground mayday that changed a safety culture, as multi-regime cases (U23). A23 and A24 took Brookhollow as the documented instance. The case is CASE-135.
From the reading, 2026-10-22. The report confirms the case’s quoted findings and the exchange’s unverified details, with one exception. It calls a mayday an “incident within an incident”; it finds that search of the burning building was not resumed for over eleven minutes after the mayday although the mayday was under control within minutes; and it finds that command did not announce the mayday’s end, as WFD policy required. The “four firefighters” is not in it. The later mayday procedure postdates it; the county’s implementation dashboard records a “COMMAND PROCEDURES - MAYDAY” chapter adopted into WFD policy by 24 October 2025. See NOTE-1073 for the bearing on CLAIM-422, CLAIM-027 and TERM-017.
